Spotlight ranks documents that mention a word. Search Everything matches file and folder names exactly, across every location you index, and answers on each keystroke. It is the tool for “where is that file called…”, and it works from any app with ⇧⌘Space.

Only the folders you add, minus editable ignore rules for caches, build output and the like. External drives and the local Google Drive, OneDrive and Dropbox folders can be included. The index is a set of files in your Library folder; it never leaves your Mac and can be deleted from Settings.

Indexing runs at low priority and pauses when you are busy. A home folder of a few hundred thousand items takes minutes the first time; after that only changes are tracked, and search itself works on names held in memory.

No. macOS shows its usual one-time prompts for Desktop, Documents and Downloads. Full Disk Access is optional, only for folders macOS otherwise refuses, and the app names the exact Settings pane if you want it.

For the hours you spend finding, moving, comparing and cleaning up, yes: four views, tabs, preview, dual pane, undo for every operation and the built-in tools. Finder keeps the Desktop and system dialogs, and Reveal in Finder is always one click away.
